Please kindly print and complete the forms prior to your child’s visit and bring them with your insurance card to your appointment.

If you are unable to print the forms, please plan to arrive 10-15 minutes early for your appointment so that you can complete them in our office.

Practice Policy and Guidelines


New families, please plan to arrive 10 minutes before your scheduled appointment. This will allow time for your child to get acquainted with the office and play in our waiting area. It will also allow extra time for you to check in and fill out any additional paperwork. A parent or legal guardian must be present in the office during the initial examination and/or any filling appointments.

We reserve our morning appointments for children under the age of six. Earlier appointments work best for younger pre-school children since they are less tired and eager to pay attention and cooperate. We are sympathetic to the demanding schedules of our school-aged patients and therefore reserve our afternoon appointments for elementary, middle and high school patients. Holidays are open to all patients however, working parents, college and high school students get first priority. We have appointments available on all Monday holidays as well as school fall, winter and spring breaks.

Re-care appointments are made six months in advance for your convenience.

Please note that your scheduled appointment is reserved specifically for your child. Any change to your appointment affects all of our patients. If a cancellation is unavoidable, please call the office at least 24-hours in advance so that we may give that time to another patient. A broken/missed appointment without 24-hour notice may result in a cancellation fee. Also, if you arrive 10-15 minutes late for your appointment, you may be asked to reschedule for another appointment time.

We try to see all patients on time for their scheduled appointment, and make every effort to stay on schedule. Please call us if you are running late to your appointment. There are times when our schedule is delayed in order to accommodate an injured child or emergency. Please accept our apology in advance should this occur during your appointment. We will provide you the same courtesy if your child is in need of emergency treatment.
